ABSTRACT

Objective:To study the early outcomes of emergency operation employed with one of the prevalent surgical methods in neonates diagnosed as pulmonary atresia with intact ventricle septum(PA/IVS) and critical pulmonary stenosis(CPS), and to analyze the risk factors related to its early results.Methods:A retrospective analysis was conducted to collect the data of neonates suffered from PA/IVS and CPS from January 2016 to January 2020 in cardiothoracic surgery department at Shanghai Children′s Medical Center.According to their Z score, which reflects the development degree of right ventricle, the neonates received one of the relevant popular operations.Early outcomes were summarized and risk factors related to its early stage mortality were analyzed.Results:A total of 65 neonates were enrolled, including 27 CPS cases.They were operated on the basis of their respective Z scores, and also according to these data, the primary surgical procedure was chosen from the following methods: Blalock-Taussig(B-T) shunt, pulmonary valvulotomy and right ventricular outlet enlargement.Totally seven cases died, and mortality was 10.7%.Nine cases received re-operation in several following days because of severe hypoxemia and low cardiac output.All patients had mild improvement in oxygen saturation after operation and relied on large dose of inotropic agent.Multivariate Logistic regression analysis showed that B-T shunt and re-operation in early period were risk factors for death in neonates with PA/IVS and CPS.Conclusion:There is a higher mortality in neonates who received emergency operation for PA/IVS and CPS.Accurate assessment of the right ventricle development degree and selecting the corresponding appropriate surgical method is critical for the optimal result.B-T shunt and early stage re-operation are the risk factors for death in neonates with PA/IVS and CPS who received emergency operation.

ABSTRACT

Objective@#To investigate the association between estimated glomerular filtration rate (eGFR) and all-cause mortality in the elderly aged 65 years and older in longevity areas in China.@*Methods@#Data used in this study were obtained from Healthy Aging and Biomarkers Cohort Study, a sub-cohort of the Chinese Longitudinal Healthy Longevity Survey, 1 802 elderly adults were collected in the study during 2012-2017/2018. In this study, the elderly were classified into 4 groups, moderate-to-severe group [<45 ml·min-1·(1.73 m2)-1], mild-to-moderate group [45- ml·min-1·(1.73 m2)-1], mild group [60- ml·min-1·(1.73 m2)-1] and normal group [≥90 ml·min-1·(1.73 m2)-1] according to their eGFR levels.@*Results@#After 6 years of follow-up, 852 participants died, with a mortality rate of 47.3%. Multivariate Cox regression analysis showed that the levels of eGFR were negatively correlated with all-cause mortality risk in the elderly (the HR of elderly was 0.993 and the 95%CI was 0.989-0.997 for every unit of eGFR increased, P=0.001), while compared with the group with normal eGFR, the HRs (95%CI) of the elderly in the moderate-to-severe group, mild-to-moderate group, and mild group were 1.690 (1.224-2.332, P=0.001), 1.312 (0.978-1.758, P=0.070), 1.349 (1.047-1.737, P=0.020) respectively [trend test P<0.001].@*Conclusion@#The decrease in eGFR was associated with higher mortality risk among the elderly in longevity areas in China.

ABSTRACT

Objective:To study the curative effect of platelet-rich plasma (PRP) combined with internal fixation with hollow screws on femoral neck fractures.Methods:The clinical data of 160 patients with femoral neck fracture were retrospectively analyzed who had been treated by internal fixation with hollow screws at Orthopedic Department Ⅲ, The Second Affiliated Hospital to Luohe Medical College from May 2012 to May 2018. According to whether PRP was used or not to assist their internal fixation, they were divided into a PRP group ( n=80) and a control group ( n=80). In the PRP group, there were 46 males and 34 females with an age of 52.3 years±7.6 years, and one case of type Ⅰ, 5 cases of type Ⅱ, 57 cases of type Ⅲ and 17 cases of type Ⅳ by the Garden classification. In the control group, there were 41 males and 39 females with an age of 50.6 years ± 7.3 years, and 2 cases of type Ⅰ, 7 cases of type Ⅱ, 51 cases of type Ⅲ and 20 cases of type Ⅳ by the Garden classification. The 2 groups were compared in terms of fracture healing time, nonunion, femoral head necrosis and Harris hip scores. Results:The 2 groups were comparable because their preoperative general data showed no significant differences ( P>0.05). The 160 patients obtained follow-up for 12 to 36 months. The PRP group showed significantly shorter fracture healing time (4.3 months ± 1.0 months), significantly lower incidences of nonunion [0% (0/80)] and avascular necrosis of femoral head [3.8% (3/80)] than the control group [7.3 months ± 1.3 months, 7.5% (6/80) and 15.0% (12/80), respectively] (all P< 0.05). The Harris scores at 6 and 12 months after operation for the PRP group (88.7±5.3 and 94.2±4.8) were significantly higher than those for the control group (81.4±4.6 and 84.2±5.2) (both P<0.05). Conclusion:In the treatment of femoral neck fractures, compared with internal fixation with hollow screws alone, platelet rich plasma combined with internal fixation with hollow screws can significantly shorten fracture healing time, reduce incidence of avascular necrosis of the femoral head and improve functional recovery of the hip joint.

ABSTRACT

Objective:To summarize cardiac function feature and its support method after ALCAPA(anomalous origin of left coronary arteries from pulmonary artery) operation. Analysis its early outcome and risk factors of its mortality.Methods:Review the clinical data of 108 ALCAPA cases treated in Shanghai Children Center between January 2005 and December 2017. All the cases were divided into two groups according to their ages when they received the surgery: group 1<1 yr; group 2>1yr. Adopted LVEF(left ventricle ejection fraction) and LVEDD(left ventricle end-diastolic diameter) Z-score as a parameter to describe the cardiac function change after operation. Summarize the selection and application of vasoactive agents and its score(VIS) after operation, indirectly reflect the post-operative cardiac function. Analysis the opportunityofinitiation and termination of mechanical circulation support and the timefor mechanical ventilationevacuation. Adopt the logistic analysis to find the risk factors of early death risk factors after ALCAPA operation. Results:Cardiac function had little improve in early period after ALCAPAoperation, did not attained normal range in fifth day post-operative, LVEF was 0.42 in group 1 and 0.45 in group 2, respectively. The application of vasoactive agents tended to choose α, β receptor-agonist, which epinephrine and norepinephrine is the preferred and VIS score was high in early period. Mechanical circulation support was used in 18 cases. 12 cases died in our group, mortality was 11%. Logistic regression analysis implied that low age and low level of LVEF before operation is the risk factor for mortality. Conclusion:Cardiac function was still in low level in early period after ALCAPA operation, depended on higher dosage of vasoactive agents and mechanical circulation support if necessary. We recommend the LVEF index for evacuated from mechanical circulation support and mechanical ventilation is over 0.40 and 0.35 respectively, and other clinical sign should be considered simultaneously. Low age and low LVEF level before operation is the risk factors for early death after ALCAPA operation.

ABSTRACT

OBJECTIVE@#To survey the residents for their understanding of Coronavirus Disease 2019 (COVID-19)-related knowledge, attitude and practices (KAP) in two hard hit provinces of China to facilitate the governmental decisions on strategies against the disease.@*METHODS@#We invited the participants from Hubei and Henan Provinces of China for an internetbased survey starting from 12:00 on February 21, 2020 to 12:00 on February 23. The survey included the general conditions, KAP of COVID-19, psychological status and living conditions of the residents.@*RESULTS@#The effective response rate of the questionnaire was 98.9%. The mean (P25, P75) age of the participants was 19 (16, 40) years, and 54.3% of them were students. Social media were the most important source of information concerning the pandemic of the respondents. The respondents had a high awareness of person-to-person transmission of the virus through the respiratory tract or droplets but showed a relatively low level of awareness of the population susceptible to COVID-19 and its specific symptoms. The results of multivariate analysis showed that women, undergraduate students (including college students) and higher degree holders had better knowledge of COVID-19 ( < 0.05); the proportion of respondents who expressed to have different levels of psychological stressed such as worry, anxiety and panic reached 77.2%; 16.7% of the responders considered psychological interventions necessary for their psychological conditions; 63.6% of the respondents confessed a bias against the people returning from Hubei and Henan provinces, while 22.4% worried that they might be biased because of their residence in Hubei and Henan. The rate of personal protective equipment shortage was as high as 69.4%; the rates of the responders who would "covering the mouth and nose when coughing or sneezing", "properly use masks in accordance with regulations", "maintain proper hand hygiene ", "avoid gatherings with relatives and friends" and "refrain from going to public places" were 92.4%, 95.9%, 93.5%, 88.8% and 93.1%, respectively. Women and groups with good knowledge of the disease reported better protective behaviors against the diseases ( < 0.05).@*CONCLUSIONS@#The residents in Hubei and Henan Provinces have generally good KAP related to COVID-19, and the online platforms plays a positive role to in circulating epidemic-related information. It is essential to further increase the supply of the protective materials and pay more attention to the mental health of the residents during the pandemic, and psychological counseling and psychological protection should be provided if necessary.

The main mechanism is to inhibit membrane transporter function, affect enzyme transfer system, inhibit anti-apoptotic pathway activation and anti-apoptotic protein expression.

Conclusion MP-MRIcombinationcanobtain moreeffectiveinformationforprostatediseases,andhashigherspecificity,accuracy, positivepredictivevalueandnegativepredictivevalueindiagnosingPCa.

ABSTRACT

Objective To explore whether using of exogenous pulmonary surfactant( PS) can im-prove recovery of infants suffering postoperative acute respiratory distress syndrome after cardiac surgery and whether kinetic analysis of pulmonary functional change can be helpful to indicate an appropriate dosing scheme. Methods Nineteen infants received an exogenous PS( Curosurf,100 mg/kg,treatment group) due to acute respiratory distress syndrome after cardiac surgery for congenital heart defects. They were compared with 24 patients without its administration despite the same postoperative complication( control group) . Oxy-genation index( OI) and ventilation index( VI) were calculated and fitted with a monoexponential function be-fore and after its use. Other outcomes including chest radiography,duration of mechanical ventilation,inten-sive care unit and hospitalization were also analyzed. Results All infants who received PS survived,whereas three infants in the control group died. The duration of mechanical ventilation and hospitalization were signifi-cantly shorter after PS administration[(21. 3 ± 9. 2) d vs. (31. 1 ± 13. 4) d,t=6. 520,P=0. 004;(30. 2 ± 13. 2)d vs. (41. 3 ± 16. 5)d,t=2. 185,P=0. 03]. The infants received either one (13 cases,subgroup A) or two doses (6 cases,subgroup B) before successful weaning from the ventilator. After the first dose was ad-ministered,the maximal rates of OI and VI change were significantly higher of infants in the subgroup A[OI:(2. 9 ±1. 7) vs. (1. 0 ± 0. 8),t =3. 012,P =0. 02;VI:(16. 6 ± 9. 6) vs. (5. 8 ± 5. 6),t =2. 980,P =0. 02]. Twelve hours after the first dose,both parameters in the subgroup B deteriorated and a second dose was administered 24 h later. Conclusion Exogenous PS is an efficient medication for infants suffering acute respiratory distress syndrome after cardiac surgery. Kinetics analysis of functional change after initial surfac-tant use may be referred for early determination of an optimal dosing scheme.

ABSTRACT

OBJECTIVE: To explore the current status and influencing factors of regulatory emotional self-efficacy(RESE) among graduate students of medical college. METHODS: A cluster sampling method was used to select 3 225 postgraduates in class of 2015 and 2016 from a medical college as research subjects. A RESE survey was conducted using the Regulatory Emotional Self-Efficacy Scale. RESULTS: The score of RESE was 66.8±7.6. The score of perceived self-efficiency in expressing emotion was 25.7±3.0. The score of perceived self-efficiency in managing negative emotion was 41.1±6.0. Multivariate linear regression analysis showed that the RESE scores of the first grade graduates were higher than those of the second-grade. The RESE scores of students whose mothers′ educational level above primary school were higher than those with their mothers′ educational level being illiteracy and below primary school. The RESE scores of those being more optimistic about job prospects were higher than those being less optimistic about job prospects. The RESE scores of those with large amount of exercise were higher than those with small amount of exercise. The partial regression coefficients were-0.65, 1.03, 4.33 and 1.87(P<0.05). CONCLUSION: The level of RESE in medical college graduate students is high. The grades of medical graduates, attitudes toward employment prospects, physical exercise and mother′s educational level are the influencing factors of their RESE.

ABSTRACT

Objective To simplify regional citrate anticoagulation(RCA) in continuous veno-venous hemofiltration (CVVH) with a calcium-containing replacement solution for children after congenital heart surgery.Methods The clinical data of 20 children with RCA in CVVH after congenital heart surgery were retrospectively analyzed.All cases were divided into two groups:the traditional group(12 cases) using a calcium-free replacement solution and the modified group(8 cases) using a calcium-containing replacement solution.The blood gas data,plasma ionic calcium,total calcium/ionic calcium ratio,circuit survival time,calcium supplement and the number of adjustments of replacement fluid formula were compared between the two groups after CVVH.Results During RCA-CVVH,serum HCO3-,pH,systemic ionized calcium,and the ratio of total calcium / ionic calcium were higher than those before RCA-CVVH,but all in normal range.There was no significant difference in circuit survival time between two groups [(50.5 ± 2.3) h vs.(48.8 ± 4.7)h,respectively,P >0.05].Calcium supplementation in the traditional group was significantly higher than that in the modified group[2.5% calcium chloride supplementation was(0.43 ±0.11) ml/(kg-h) in the traditional group and(0.13 ± 0.17) ml/(kg-h) in the modified group].The number of replacement solution adjustment was (2.7 ± 1.1) times in traditional group,while the modified group did not change the replacement formula.Conclusion RCA using a calcium-containing replacement fluid in hemofiltration,can reduce the supplement of calcium and the times of the replacement solution adjustment,which is easy to operate.It can be used safely,effectively and conveniently in CVVH children after congenital heart surgery.

ABSTRACT

OBJECTIVE@#To observe the effectiveness of acupuncture on claustrophobia, and to explore the effects of acupoint specificity on claustrophobia.@*METHODS@#This was an evaluator-blinded randomized controlled trial. One hundred and sixty patients who presented with claustrophobia during magnetic resonance imaging examination were randomized into an acupoint group, a non-acupoint group, a sham-acupoint group and a blank group, 40 cases in each one. The patients in the acupoint group were treated with acupuncture at Zhaohai (KI 6), Taichong (LR 3), Lingdao (HT 4), Neiguan (PC 6), Shenmen (HT 7), Danzhong (CV 17), Baihui (GV 20) and Fengchi (GB 20). The patients in the non-acupoint group were treated with acupuncture at points 0.5 next to the acupoints above. The patients in the sham-acupoint group were treated with acupuncture at acupoints not closely correlated to claustrophobia in corresponding segment. All the acupuncture was given once. No treatment was used in the blank group. The state anxiety questionnaire (S-AI) was observed in all the patients at the end of MRI examination before and after treatment. The clinical therapeutic effects were compared among four groups.@*RESULTS@#Compared before treatment, the S-AI score was reduced in the acupoint group, non-acupoint group and sham-acupoint group after treatment (0.05). After treatment, the S-AI scores in the acupoint group, non-acupoint group and sham-acupoint group was lower than that in the blank group (<0.05, <0.01), and the differences of S-AI score were higher than that in the blank group (<0.01). The S-AI score in the acupoint group was lower than that in the non-acupoint group and sham-acupoint group (<0.05), and the difference of S-AI score was higher than those in the non-acupoint group and sham-acupoint group (<0.05). The difference of S-AI score in the non-acupoint group was higher than that in sham-acupoint group (<0.05). The total effective rate was 92.5% (37/40) in the acupoint group, which was significantly superior to 25.0% (10/40) in the non- acupoint group, 17.5% (7/40) in the sham-acupoint group and 5.0% (2/40) in the blank group (<0.05, <0.01).@*CONCLUSION@#Acupuncture showes superior effect on claustrophobia, and its tranquilizing effect may be related with acupoint specificity.

ABSTRACT

Thromboxane A2 receptors (TPs) widely distribute in different organ systems and localize both on cell membranes and in intracellular structures.TPs are the members of seven-transmembrane G-protein-coupled receptor (GPCR) super family.Historical-ly, the involvement of TPs in platelet functions has received the greatest attention .TPs have the capacity to activate different signaling cascades which regulate platelet shape change , aggregation and secretion response .Currently , anti-platelet drugs primarily act on re-ceptors and /or signaling molecules in activation pathways .The signaling transduction of TPs in platelet contributes to the investigation of the effects of extracts of traditional Chinese medicine on antiplatelet aggregation and the exploration of the action mechanisms .

ABSTRACT

Objective To study the different drying technology.on Wudang Lonicerae Flos (Ⅰ) in chlorogenic acid content.Methods Using HPLC method to research the difference in the contents of chlorogenic acid of Wudang Lonicerae Flos (Ⅰ) employing oven-drying method,air-dried method and steamed drying technologies.DIONEX C18 (4.6 mm×250 mm,5 μm) was used.The mobile phases consisted of acetonitrile-0.4% phosphoric acid aqueous solution (17 ∶ 83).The detection wavelengths of chlorogenic acid was 327nm.The flow rate was 0.4 ml/min.The column temperature was 30 ℃.Results The Separation of Chlorogenic acid content was satisfactory.The concentration and peak area showed a good linearity relationship with the range of 0.285-2.850 μg.Precision,repeatability and stability met the requirements.The average recovery was 100.26% with RSD 1.95%.Chlorogenic acid content in the steamed drying Wudang Honeysuckle (Ⅰ) was higher.Conclusions The method of steam drying is better than that of oven-drying method and drying.

ABSTRACT

Objective Grey red-backed voles (Myodes rufocanus) are agile, fierce and hard to catch, thus, it is difficult to judge their gender by external appearance, especially for the juvenile voles. Therefore, it may cause difficulties to their allocation and later breeding in laboratories. The aim of this paper is to establish a rapid, simple and accurate method for gender identification of grey red-backed voles. Methods Fresh hair follicles were taken from 6 adult male voles, 3 adult females and 14 4-week-old juvenile voles, 5 male and 5 female 9-week-old Wistar rats, and 5 male and 3 female 6-week-old BALB/c mice. The genomic DNA was extracted using Chelex-100 resin and the zinc-finger Y/X gene (ZFY/ZFX) and the gene of sex-determining region of the Y (SRY) chromosome were amplified by PCR, and a double PCR amplification method was established. Results The ZFY/ZFX gene and SRY gene were simultaneously amplified from the male voles, while only the ZFY/ZFX gene was amplified from the females. The gender of all 23 voles, 10 Wistar rats and 8 BALB/c mice were correctly identified with this method, and the PCR results were consistent with the phenotypic and autopsy results. Conclusions Using fresh hair follicles as experimental materials for gender identification of grey redbacked voles can alleviate shock and damage to the animals. The established double PCR amplification method is accurate, simple, rapid, and deserves to be used for gender identification of grey red-backed voles.

ABSTRACT

Objective To investigate the early curative effect of Blalock-Taussig(B-T) shunt and summarize the key point in its post-operative intensive care,analyze the related risk factor of its mortality.Methods We collected 71 patients who received B-T shunt in Shanghai Children's Medical Center from June 2015 to June 2017. All of 71 cases were divided into neonates group (25 cases) and non-neonate group (46 cases). All patients were operated through median sternotomy,19 were emergency cases,in which 15 cases were neonates. Eight cases used cardio-pulmonary bypass in our study,in which 4 cases were neonates. Twenty-nine cases received ductus ligation in total 34 patients with patent ductus arterious. Percutaneous heparin was immediately administered after operation as an anticoagulation therapy. Relative large dosage inotropic agents was used after sufficient volume infusion in order to guarantee diastolic pressure higher than 35 mmHg(in neo-nates >30 mmHg,1 mmHg =0. 133 kPa). Ventilator parameter was adjusted to obtain appropriate flow distribu-tion between pulmonary and systemic circulation. In order to avoid excessive pulmonary flow,the target PO2 was set between 40 ~ 45 mmHg. Results Oxygen saturation elevated by 15% after operation in all 71 cases. Average mechanical ventilation time was 92 hours,mean inotropic agents score was 17. There were 6 dead cases,mortality was 8. 5% . In neonates,4 cases dead,mortality was 16. 0% . There was no significant difference in ventilation time,ICU retention day and mortality between neonates group and non-neonates group. The logis-tic regression analysis showed that the body weight and ratio between the shunt conduit diameter and body weight (mm/ kg) were the risk factors for early post-operative mortality[body weight:2500 ~ 3500 g:OR =6. 729,95% CI =1. 373 ~17. 131;≤2500 g:OR = 7. 832,95% CI = 2. 563 ~ 28. 325. the ratio between the shunt conduit diameter and body weight (mm/ kg):1. 10 ~1. 15:OR = 4. 204,95% CI = 1. 195 ~ 23. 926;≥1. 15:OR= 3. 981,95% CI =1. 195 ~12. 926]. Conclusion B-T shunt is still a high risk operation although it is a palli-ative method. Low body weight and bigger diameter conduit are the significant factors related to its mortality. Strict control of the systemic and pulmonary flow ratio is the key point in the post-operation intensive care.

ABSTRACT

Objective To analyze the influencing factors of surgical management for Blalock-Taussig shunt during neonatal period,and to summarize the clinical experience,therefore,to improve the surgical outcome.Methods The clinical data between Jan 2011 and Dec 2016 were reviewed,42 neonates(26 males,16 females)with the mean age from 1 to 29 days,and weight from 2.3 to 4.1 kg,underwent a Blalock-Taussig shunt.The additional operation included PDA retention in 3 patients,right ventricular outflow tract retention in 12 patients,B-T conduit banding in 2 patients and PDA banding in 1 patient.Results The early mortality was 28.6% (12/42).Univariate analysis revealed low birth weight,waiting time for surgery,preoperative acidosis or cardiac shock,dual pulmonary blood supply,conduit/weight,high IS (inotropic drugs score),unplanned intervention as risk factors for early death.In the multivariate analysis,preoperative acidosis or cardiac shock,conduit/weight,high IS were independent risk factors of early death.Conclusion The mortality rate after the neonatal modified Blalock-Taussig shunt remains high.It can be improved by proper perioperative treatment,immediate surgical treatment and choose suitable conduit size,maintain the stable circulation.

ABSTRACT

Objective To optimize the flash type extraction ofDanzhuye Granules.Methods With the total flavonoids and extract yield as indices, the orthogonal test was adopted to optimize the extraction times, the ethanol concentration, solid-liquid ratio, and the duration of extraction forDanzhuye Granules.Results The factors that infuenced the extract efficiency from high to low were the extraction times, volume fraction of ethanol, the ratio of liquid to solid, and the extraction time. The optimum extracting condition included that the origin was extrated by the ethanol volume fraction 80% with the material to liquid ratio 30:1, total of 3 times, with each time of 3 minutes. Validation experiments of the potimal condition showed that the extraction of total flavonoids was 29.1 mg/g, and the extract yield was 23.6%.Conclusions The flash type extraction method is suitable for extraction, which provides a new method for the development and utilization ofDanzhuye Granules.

ABSTRACT

Objective To establish a method for the determination of borneol, isoborneol, menthol in Bingshuang-Meisu pill.Methods The capillary column PEG-20M(30 m × 0.32 mm, 0.25μm) was used, naphthalene was adopted as internal standard substances, split injection(split ratio 10:1), temperature programme, the injector temperature was 240℃, the detector temperature was 250℃.Results The linear response range of menthol, borneol, isoborneol were 13.00-104.00μg/ml, 6.80-54.40μg/ml, 5.40-43.20μg/ml and the average recovery were 97.90%, 98.05% and 97.77%, RSD were 0.98%, 1.53% and 1.40% respectively. Conclusion The method was highly sensitive, accurate, and repeatable. It can be an important method to control the standard ofBingshuang-Meisu pill.

ABSTRACT

Objective To establish the HPLC method for the determination of chlorogenic acid and luteoloside in Jinmei-Qingshu granule. Methods The DIONEX C18 (250 mm×4.6 mm, 5 μm) column was used, the mobile phase consisted of acetonitrile:0.4%H3PO4, gradient elution, the flow rate was 0.8 ml/min, the column temperature was 30 ℃, and the detecting wavelength was at 330 nm. Results The cablibration curve was linear within a range of 14.10-282.00 μg and 0.60-12.00 μg, the average recovery of this method was 98.87%, 97.75% and the RSD were 1.44%, 1.52%, respectively. Conclusions The method was highly sensitivity, accurate, repeatable and with high specificity. It can be an important method to control the standard of Jinmei-Qingshu granule.
